2)?投標(biāo)人必須是制造商，本項(xiàng)目不接受代理商投標(biāo)；
3)?投標(biāo)人必須是本裝置專利商推薦的供貨商，或在投標(biāo)前取得本裝置專利商出具的合格供貨商認(rèn)證材料（需提供本裝置專利商出具的合格供貨商認(rèn)證材料）；
4)?投標(biāo)人具備10萬噸/年及以上規(guī)模LDPE或EVA同類裝置、同類設(shè)備供貨、應(yīng)用業(yè)績。投標(biāo)人需提供業(yè)績表，證明有相似工況下設(shè)備的制造經(jīng)驗(yàn)（標(biāo)明型號、用戶名稱、略在地、投用時(shí)間、裝置規(guī)模等），并出具銷售合同關(guān)鍵頁或最終驗(yàn)收證明復(fù)印件。若投標(biāo)人投標(biāo)文件提供略出具的合同或最終驗(yàn)收證明復(fù)印件不略要求，則視為沒有業(yè)績；
5)?投標(biāo)人應(yīng)當(dāng)提供在開標(biāo)日前3個(gè)月內(nèi)由其開略資信證明的原件或復(fù)印件；
6)?投標(biāo)人在中國境內(nèi)近三年沒有不良業(yè)績記錄或嚴(yán)重質(zhì)量問題，否則其投標(biāo)也將被否決；
7)?本項(xiàng)目不接受聯(lián)合體投標(biāo)。
